Conventional Liposculpture

It is perhaps the most widely performed procedure of all today. It allows to shape the body contour, removing undesirable fat deposits. There are synonyms for this same procedure that can cause confusion, but ultimately refer to the same procedure, such as lipoplasty, and liposuction.

Breast augmentation

Breast augmentation is designed to increase the volume of small breasts or those that due to weight loss or after pregnancy have lost their previous appearance. Technically it increases the volume but it is not designed to raise them or change their shape or position and this should be very clear when discussing it with the surgeon to avoid false expectations and lack of satisfaction. Getting started
The first appointment is with one of our staff members to review your medical history and to get an idea of your desires. This appointment is scheduled at a time outside of normal office hours to allow enough time for meaningful dialogue and fitting of different sized implants in a relaxed atmosphere.

There is no charge for this visit. With many years of experience using this approach, we have found that a consult with a staff member provides for an excellent exchange of information, and sets the stage for an informed patient.

The many aspects of the operation are discussed including a discussion of pain, risks and potential complications, the pros and cons of saline versus silicone breast implants, benefits of placing the implants under the muscle, need for a breast lift and the timing of this, and most important the proper sizing of the breast implants. Before surgery and post-surgery photos are reviewed to show a wide variety of breast shapes and sizes for patients to get a sense of the possible outcome and results. Patients may return for a second visit, also at no charge, if there are remaining questions.

Once the patient has a good understanding of breast enlargement surgery, she is ready for a consult with our plastic surgeon. Measurements are taken, and this visit is more for “fine-tuning” to focus on the desires of the patient and what needs to be done to fulfil her dreams. Our goals are to allow the patient to direct her destiny rather than force the patient into a particular pre-conceived mold. Everyone is different and their needs vary among patients.

By fine-tuning the surgery, we are able to give the patient what she truly desires. Often the patient’s partner is present so that any concerns or thoughts can be addressed. The consultation fee is applied to the surgery and there is no charge for a repeat consultation.

Pain control

A major concern of patients is how much pain they can expect. They have friends who tell them that the surgery is very painful. We have found the contrary to be true in that most of our patients have very little pain (more of a discomfort) or almost none at all.

Our plastic surgeon developed the concept of pain control catheters placed at the time of surgery so that patients can control the instillation of a long-acting local anaesthetic. This technique was developed in the 1990s and published. Pain control catheters continue to be a mainstay of our post-procedure care. To date, more than 1,000 patients have benefitted from this device without any additional complications.

There is no extra charge for the indwelling pain control catheters.

Added to this are careful post-operative instructions regarding the need to move the arms, showering the night of the surgery (there is no problem with the catheters), getting dressed, going out that night for supper and quickly getting back into a normal routine.

When combined with pain control medications, most patients are amazed at how comfortable they are after the surgery. The pain control catheters are usually removed the next day in the office, though some patients elect to keep them an extra day.

Scars

The scar is placed in the shadow of the breast fold and usually heals quite nicely. The length of the scar is small when a saline implant is used and longer for silicone implants.

With over 2,000 breast enlargement surgeries performed since the 1970’s, it is rare to see a poor scar placed in the mammary crease.

How long do breast implants last?

The lifespan of the newer silicone implants is not known. It is expected that they will last at least 20 years given the many improvements in the manufacture of these implants. Saline implants also have a long life span.

It is important to “overfill” these implants to diminish wrinkling which can weaken the envelope and cause leakage.

“Under’s or over’s”

Most implants are placed under the muscle which gives a softer more natural feel to the implants and fewer complications. I prefer a breast fold incision for either saline or silicone gel breast implants.

Follow-up care for our breast enlargement patients

Post-operative patients are followed without charge.

Appointments are made after the surgery to teach breast mobility exercises and removal of pain control catheters. Suture removal is not needed because dissolving sutures are used.

Further, follow up appointments are generally done at about four months and again a year later, then as needed. Beyond one year very little follow up is required.

Outcomes

Breast enlargement surgery is one of the most satisfying operations performed. There is a very low rate of complications and patients are generally very happy with their appearance.

“Touch up” procedures are almost never necessary because of the amount of time that was spent choosing the correct implants.

Complications

Complications from breast enlargement surgery are rare. It is always best to read your permit carefully and ask any questions to be sure you are properly informed.

Though rare, any patient may require a trip back to the operating room because of bleeding.

Since the 1970’s in over 2,000 patients no patient has required a blood transfusion. Infection is very uncommon. Distortion of the breasts is almost never seen.

Three types of outpatient surgery are possible: Surgery can be done in a hospital as an outpatient, in a surgi-center, or a physician’s office.

Usually, only minor procedures such as removal of skin lesions are done in a doctor”s office, more major procedures usually require the support systems of a hospital or a surgi-centre. Currently, most plastic surgeons prefer surgi-centres for operations that do not require an overnight stay.

Surgi-centres are further divided into those that can perform surgery under local anaesthesia only, and those that can use general anaesthesia. A certified surgi-centre is one that gets examined on a regular basis by an approved agency to maintain the highest standards possible.

Our own certified surgi-centre, Plastic and Maxillofacial Surgery Clinic has been in existence for over 2 decades, and we have passed full certification to use general anaesthesia every two years since then.

Key personnel are advanced cardiac life support certified. The intervening year between on-site certification requires written compliance with published standards, as well as peer review of charts. Requirements change every year with the focus on increased safety for the patients and continued higher standards. 

To accomplish the safety of surgery in an out-patient setting, only healthy patients for appropriate surgeries are scheduled. Patients in poor health, or for long complicated procedures requiring one to several days of hospital observation are better off in a hospital. In our own practice, any patient can choose to have their surgery in a hospital.

Most plastic surgeons prefer an out-patient surgi-centre setting for a variety of reasons: increased privacy for the patients; a less “intense” environment; staff that is personally hired by the physician, thereby maintaining a high level of expertise in plastic surgery; and a predictable flow to the surgical schedule without interruption for emergency or urgent cases.

Patients seem to be less frightened in a certified surgi-center, making the entire experience more comfortable. Further, a much smaller ratio of patients to recovery room is common allowing for more personalized care.

Our own experience with thousands of operations has been very favourable. Apart from a small percentage of non-life-threatening complications that could occur in any setting, most patients have their surgery, leave a few hours later with verbal and written instructions, and are routinely called the evening of surgery.

The entire staff gets to know the patient, from their pre-op consultation, through the surgery, and their post-op care.
